Note from the Nurse:  Important Registration Information

·        Please ensure that the school has correct contact information for you AT ALL TIMES. Please review the list of over the counter medications that are available to be given to your child while at school. You may review these during the registration dates listed below and check the ones that you approve your child to receive.

·        Notify the nurse if your child has a chronic medical condition, will require daily medications to be given at school, will need special considerations, or has immunization exemptions.

·        All medications to be dispensed by the nurse must be brought in by an adult.  DO NOT SEND MEDICATIONS WITH YOUR CHILD TO SCHOOL!  Permission forms must be signed by your child’s doctor PRIOR to providing the medication to the school nurse.  Medications are not allowed to be administered at school without doctor’s permission. Permission forms are available on the SDOC website to print, and hard copies will be available at the main office at Walhalla High School Monday-Thursday 8am-4pm during the summer for your convenience.  Please plan to take this form with you to your child’s doctor’s appointment.

·        The nurse will be available at the school on August 1st 8:00-5:00 and 2nd from 10:00-6:00. Please use this time to provide medications to the clinic and discuss any needs or concerns you may have.  The first day of school is quite hectic, and I will most likely be occupied with student health needs.

Thank you for your cooperation, and I look forward to a healthy 2018-2019 school year!
